If you notice any missing or incorrect information about a provider, please click on the provider’s listing and select 'Report Incorrect Information'.

Find a Vision Provider - To locate a vision provider, you can search directly by name or by choosing ‘Ophthalmology’ or ‘Optometrist’ under the Specialty field to see a full list of vision providers in the network.

To request a hard copy of Blue Advantage’s Provider & Pharmacy Directory, please call our Customer Service Department toll-free at 1 (866) 508-7145. TTY users should call 711. Our phone lines are open 8 a.m. to 8 p.m., 7 days a week from October – March and 8 a.m. to 8 p.m., Monday – Friday from April – September.

Blue Advantage will mail a hard copy of the Provider & Pharmacy Directory to you within three (3) business days of your request. Blue Advantage may ask whether your request for a hard copy is a one-time request or if you are requesting to receive the directory in hard copy permanently. If you request it, your request for hard copies of the provider directory remains until you leave Blue Advantage or request that hard copies be discontinued.
