Blue Advantage from Blue Cross and Blue Shield of Louisiana is an HMO plan with a Medicare contract. Blue Advantage from Blue Cross and Blue Shield of Louisiana is a PPO plan with a Medicare contract. Enrollment in either Blue Advantage plan depends on contract renewal.

Please note that as a Blue Advantage member, you must continue to pay your Medicare Part B premium. Enrollment in a Blue Advantage plan may be limited to specific times of the year. Blue Advantage (HMO) members must use Blue Advantage network providers except for select situations such as emergency care. Blue Advantage (HMO-POS) members can also use out-of-network providers for an additional cost when prior authorization is obtained.
